  • When did the problem start? Please include date and time zone specific to your location. What have you done to try to resolve the issue already? Can you replicate the issue? If so please provide specific steps you are taking to replicate the issue. If this is File request, what is the name of the file, and where should it go? If this is a hardware device approval, what device is it, why is it being used, and who authorized it.

  • Emergency: The issue has a company or department wide impact.

    High: The issue is preventing you from completing today’s work.

    Medium: The issue is hindering your production, yet today’s work can still be accomplished.

    Low: The issue is a “want” or request that can be done eventually.
